Completing the 9-12 Grade Submission Form and Checklist

This article walks through completing a Grade Submission Form and Checklist for 9th - 12th grade students. Grade portfolios should be completed at the end of every semester for students in grades 9th - 12th and submitted to [email protected] for 9th - 11th grade students or [email protected] for 12th grade students at the end of every semester or at the end of your student's school year. There is no deadline for submitting grade portfolios. 

 

Note: Senior students should submit their Grade Portfolio as soon as possible to ensure they can be processed quickly for mid-year reports (college applications) and post-high school needs.

Completing the 9-12 Grade Portfolio Submission Checklist

Record the student and parent names, the student’s grade level, the school year, phone number, address, and email address. Be sure to check the corresponding semester.

 

Note: Final report cards will be sent back to the parent's email address as written on the Grade Submission Form.

 

Check the box that indicates you are submitting the completed Grade Submission Form with grades and that you have signed the Grade Submission Form. If filing the Attendance Record, required only for our families living in California, check the box that indicates that this is included.

 

 

When you get to the checklist, check the box next to each subject for which you are including samples.

 

 

Note:  Do NOT include course grades on this form.

 

Completing the 9-12 Grade Submission Form

On the left side of this page, you may choose to provide a brief description of your student's work in music, art, physical education, or another non-academic subject you registered your student in. Descriptions should cover the activity, frequency, and duration of the activity. They should add up to at least 60 hours per semester to earn high school credit, e.g. "Student attends piano lessons two times a week for 60 minutes each lesson, and practices six times a week for 30 minutes each session." You will check the 60+ hours/1 semester credit if you are reporting at the end of the semester or 120+ hours/1 year credit if you are reporting both semesters at the end of the academic year.

 

You may also record attendance for each semester, however, this is not required. 

 

 

Note: If you report your student's music, art, or physical education grades in the Schoology courses you should not include this non-academic information here. ⚠ Double-reporting may lead to recordkeeping errors. ⚠ 

 

On the right side of this page, you will record your student’s grades for the corresponding semester(s) using the provided grade scale.

 

 

There are reserved lines for TheologyEnglishLiteratureHistoryMathematicsScienceScience Lab, and Foreign Language.

 

There are also three lines reserved for academic electives and below this are four lines reserved for PE and any non-academic electives that you wish to report. Non-academic courses must be P/F.

 

To report science labs, a separate grade should NOT be given for the lab work but should be incorporated into the overall grade given for the course. Parents may determine the lab component's weight on the final grade, but typical values range from 15-25% of the total grade. Two written lab samples are needed per semester for lab credit on the transcript. These lab samples may be informal or formal lab reports. On the Grade Submission Form, you will use an "X" to indicate that your student is seeking the "with Lab" designation.

 

Example:

 

In the column to the left of the course title, you may indicate any special designations that pertains to the course, such as “K” for Kolbe Core, “H” for Honors, “S” for summer courses.

 


At the bottom of the page, you will sign and date the report card. A digital signature is acceptable.

 

 

Completing the Attendance Record

Record the student’s name and the academic year.

 

 

If you are keeping daily attendance records you may check the box each day that corresponds to month and date.

 

 

There is a column on the right for recording the number of days in attendance each month, and at the bottom for recording the total days in attendance for the year.

 

 

Sign and date the attendance record.

 

 

Completing Honor Cover Sheets

If you are reporting honors courses to Kolbe, you must include an honors cover sheet for each honors course.  Please refer to the article Completing Homeschool High School Honors Grade Reports for detailed instructions. Link coming soon!

 

Submitting a 9-12 Grade Portfolio

Once you’ve completed the Grade Portfolio Submission Checklist, Grade Submission Form, compiled the necessary samples (two samples of student completed work that is parent graded, per subject, per semester) and attendance record (required if you live in California and have completed the 2nd semester), you must submit your packet to Kolbe for review. Email 9-11th grade portfolios to [email protected] and 12th grade portfolios to [email protected].
